For a confirmation of submission of this form,
please read the instructions at the bottom of the form.

Please note new categories for Campaign Sight First II Pledges (34a - 34d).

July 1, 2007 through February 29, 2008
Awards will be presented at the Spring Convention on April 19, 2008

The contest is designed to focus on the goals established by the District Governor for Club and District growth and development.  All clubs are automatically entered.  This is not a requirement; however, if a club uses these rules as a set of goals for their club, it will be an outstanding club.  Scoring will be based on the information submitted on the monthly tally sheet substantiated by activities listed on the M & A report.  Points not submitted as earned can't be credited and points missed are points lost.  M & A reports received after the 10th of the month will forfeit points earned that month.  Governor's plaques will be awarded for each category:  Class A - over 35 members, Class B - 24 to 35 members, Class C - under 24 members.  This membership is based on International data as of February 29, 2008.
