Monthly Membership and Activities Report

Please read the instructions at the bottom of this form...they explain how to get a confirmation of your submission.

The Monthly Membership Report (MMR) to Lions International must be filed online by the last day of the current month.
For those who file a paper MMR, they must be submitted to International by the 20th of the current month.
This will allow the report to contain current monthly data, rather than being a month behind.

Please click the submit button only once. It may take a few minutes to transmit the data, depending on your internet connection speed. Copies of the report are automatically sent to the District officials who are to receive a copy.

When the data has been entered, a confirmation screen will appear that shows what was entered. A copy of the record can be printed by clicking "print" at the top of the web browser screen or by right clicking the mouse button and choosing "print."
